Service cutting in and out constantly

I brought my Verizon iPhone 5 over to T-Mobile over a year ago.  The service was great and my wife and I were having no problems at all until the update T-Mobile did back in September or October.  I used to get service fine at my work and great in my house.  Now my phone is constantly going between 4g to edge to "No Service" for no apparent reason.  I've tried troubleshooting all of the options that are on the Apple website for getting service to come back on like switching to airplane mode and all that, but it cuts out within 30 seconds of turning back on.  I've taken the phone into a T-Mobile store and got a new sim card and that didn't help.  I've reset the network settings and that did nothing.  Finally I restored the entire phone, completely fresh and it still didn't solve the problem.  Is there any idea to why all the sudden this would start happening?  The last thing I want to have to do right now is buy a "T-Mobile" phone when the phone I have isn't even two years old and was working perfectly fine before.  Any other things to try or solutions for the problem would be great!
