Retrive My RMA

I have 2 devices to trade in for EFT Reimbursement. I saved one RMA but lost the other. The value for both devices is $0. I only bought one new device from T-Mobile and used an old T-mobile device. i have a couple of questions:

1. Do I need to trade in both devices to be get EFT Reim.

2. Do I need to buy a second new phone to be eligible. (I will be charged ETF for 2 devices)

3. How can ii retrive the second RMA.
